A Clinical Analysis of Acute Renal Failure Accompanying Rhabodomyolysis



Abstract
Five patients with acute renal failure accompanyng rhabdomyolysis were studied and folowing results were obtained.
1) The patient consisted of 4 males and 1 female with mean age of 32.2¡¾11.9 years.
2) Rhabonyolysis was 19% cause of acute renal failure at Ewha Womans Unkiversity Hospital and trauma was the main etiologic factor of rhabdolmyolysis.
3) Serum creatinine phosphatase, serum myoglobin, orthotolluidine test of urine were useful diagnostic method in rhabdomyolysis.
4) Initial increase in hematocrit indicated volume contraction and during the clinical ourse, blood chemistry showed sere hyperkalemia, hyperphosphatemia, hyperuricemia and hypocalcemia.
5) All five cases were oliguric acute renal failure. Duration of oliguric was 7.8¡¾2.3 days and average 2.6 times of hemodialysis were required during the clinical course. 1 case expired during the clinical course.
With above results, in patients who develops acute renal failure, especially in crushing injury, rhabdomyolysis should be considered as an etiologic factor.
